By submitting a photo you:

Waive any rights of privacy, intellectual property or any other legal rights that may prevent Johns Hopkins University from using the images, and understand that IGTC has the right to screen, refuse or remove any content.
Grant permission to IGTC, in its sole discretion, to edit the photo, and to use the photo in whole or in part, in conjunction with other images, graphics, text and sound in any way whatsoever and without restrictions to the fullest extent allowed by law. These photos may be used in websites, brochures, social media, etc. and the use will be unrestricted.
Understand that you will not be compensated for use of the photo.
Release, indemnify and hold harmless Johns Hopkins University and its trustees, officers, employees, and agents from and against any and all liabilities, damages, and causes of action arising out of or in connection with the photo.

I certify that I (1) have not served as an employee, consultant or advisor of a tobacco company within the last 10 years; and (2) have not received any funding in any form from a tobacco company. Tobacco companies are defined as those companies or their affiliates that produce, market or sell products containing tobacco.
